Why there's a consult first
Phentermine is a prescription stimulant and controlled substance. It isn’t appropriate for everyone, and it’s only dispensed after a licensed provider reviews your health history and confirms it’s a safe fit. The $75 consult fee applies whether or not you’re ultimately approved — it covers the provider’s evaluation only. If you move forward, your treatment plan is billed by Optimal Wellness and your medication is billed separately by Boulevard Pharmaceutical Compounding Center.

Is the $75 the total cost, or is there more?
The $75 covers only the provider consult, paid to Optimal Wellness. If you’re approved and move forward, you’ll pay Optimal Wellness separately for your treatment plan ($225 for 3 months, or $105/mo), and Boulevard separately for the medication itself ($99 for a 90-day supply, or $75 for a 30-day supply).
What if I'm not approved at the consult?
The $75 consult fee still applies — it covers the provider’s time and evaluation, not any treatment or medication. If phentermine isn’t a fit, your provider can talk through other options.
Do I have to commit to 3 months?
No. The 3-month plan ($225 to Optimal Wellness + $99 to Boulevard = $324) is discounted versus paying month-to-month ($105 + $75 = $180/mo, or $540 over three months). You can start month-to-month and switch later.

Every prescription is compounded and dispensed by Boulevard Pharmaceutical Compounding Center at 149 Shrewsbury Street, Worcester, MA 01604 — and billed by Boulevard directly.
